click on the question mark (?) next to the Login Name field
enter the email address associated with your account
click on the box to the right of "Check box & enter code" – a four digit number will appear – enter that number in the box next to it
click the Submit button – an email will be sent to you with instructions to reset your Login Name

If you have forgotten your Password:

click on the question mark (?) next to the Password field
enter either your Login Name or the email address associated with your account
click on the box to the right of "Check box & enter code" – a four digit number will appear – enter that number in the box next to it
click the Submit button – an email will be sent to you with a link, and instructions, to reset your Password
