Death Certificate Look-ups
Non-Certified Copies for 1916-1947 Only *
 
ISGS has established this service to assist members and researchers who do not have easy access to the Illinois State Archives to obtain death certificate copies.
 
NOTE:  ISGS cannot do death certificate lookups for records found in the 1951-1971 database as those records are not in the State Archives.
 
 
NEW Death Certificate Look-up Procedure
 
We have changed how to order a death certificate!  Members, you MUST log-in to receive the discounted price of $6.00 per certificate. Non-members may order certificates for $10.00 per certificate. If you would like to join ISGS to get the discount, please visit our Join/Renew page first. Memberships take approximately 24-48 hours to process. Once you receive your "welcome letter" with your log-in information, you may place a Death Certificate order.
 
 
Two Ways to Order & Pay
 
•  By Mail with a check: place your order and print the invoice. Download the Death Certificate Order form and complete it using the Cyberdrive Illinois information. Mail the form and invoice along with your check to ISGS.
 
•  Online: place your order and pay with a credit card through PayPal. Then send an email to isgsdcorders@ilgensoc.org to include the following:
 
-- your full name, mailing address and phone number
-- do a copy/paste of the information for each death certificate you are ordering from the Cyberdrive website into the body of the email. Include the header and ONE name only such as:
 
If you are ordering multiple certificates, add additional names with the header. They can be in the same email. Then hit Send!
